In-home Services

SPHP provides these medically supervised services to the patient at home:

Observation and assessment
Skilled nursing care
Pain and symptom management
Caregiver education
Personal care
Assistance with activities of
daily living
Physical, Occupational, and speech therapy services
Guidance on advance directives and other planning
Emotional and spiritual support
Bereavement support and grief counseling

Our nurses are on-call 24/7/365 for our patients. St Paul Hospice & Palliative consists of intermittent visits made according to the needs of individual patients and his or her family.

Diagnoses

Some common diagnoses that are included in hospice care include the end stages of the following diseases:

ALS
Cancer
Dementia/ Alzheimer’s
Liver disease
HIV
Kidney disease such as failure
Cardiac and pulmonary diseases such as CHF/COPD
Neurological disease
Stroke

Those who are experiencing severe multi-symptom decline and stopping aggressive treatment, may also be eligible for hospice.
